Therapy Agreement
Introduction and Nature of Services
Thank you for choosing Dr Hannah Jenkins for your therapy needs. This document outlines our therapy agreement, including important information about services offered, confidentiality, session structure, and your rights as a client.
Confidentiality & Storing Your Data
Your privacy is of utmost importance. All information shared during therapy sessions is confidential and will not be disclosed without your consent. However, as a mental health professional, I (Dr Hannah Jenkins) have a duty of care that means if I have concerns about your safety or that of a vulnerable adult or child, I will disclose limited information of this concern to your next of kin and/or other services (e.g. GP/ safeguarding team). Concerns may also be discussed as part of clinical supervision. Information will be disclosed if legally obliged e.g. if a serious crime has taken place or court orders requiring the release of records. I will always aim to discuss all safety concerns with you first, before disclosing information to others, although this is not always possible.
It is agreed that neither party will record any of the sessions with any type of recording device unless written prior agreement of both parties has been given.
We use WriteUpp, a certified practice management software, to manage client information, appointments, and clinical notes. WriteUpp is GDPR compliant and employs robust security measures to protect and store your personal data.

Therapy Sessions
Therapy sessions are typically 50 minutes and are scheduled on a weekly or fortnightly basis unless otherwise agreed upon. Consistency and regular attendance are crucial for effective therapy. As part of treatment, there may be agreed upon tasks and skills to try out in between sessions to help reach your goals.
Fees and Payment
The cost of a 50 minute individual therapy session is £120. This may be adjusted if we agree to have a longer or shorter session for a clinical reason. Family therapy sessions incur additional charges.
Payment is due at the time of each session. Accepted forms of payment include credit/debit cards and online transfers (we do not accept cash). You can pay online through the online booking calendar.
If you are using a private health care plan to pay for your treatment, therapy will not begin until I receive authorisation from the private health care organisation. Health insurance providers do not pay for missed or cancelled sessions. The outstanding fee will be charged to the stored card, as per your Insurance Agreement.
If payments are not made on time and as agreed, Dr Hannah Jenkins reserves the right to end treatment sessions.
Cancellation Policy
If you need to cancel or reschedule an appointment, please provide at least 48 hours' notice.
Cancellations with less than 48 hours' notice will be charged the full session fee. Missed appointments will also be charged at the full rate.
Communication
You may contact Dr Hannah Jenkins via phone or email for non-urgent matters (e.g. scheduling issues). She will make every effort to respond to emails and calls as soon as possible, however only works 1 day a week in private practice.
For emergencies, please call 999 or go to your nearest accident and emergency department.
The therapeutic relationship is strictly professional. Any form of personal, social, or business relationship between you as the client and Dr Hannah Jenkins is prohibited and is against professional ethical codes of practice.
Termination of Therapy
You have the right to terminate therapy at any time. It is recommended to discuss your decision with Dr Hannah Jenkins to ensure a proper conclusion to your therapy process. Dr Hannah Jenkins may also terminate therapy if it is determined that therapy is no longer beneficial or if there is a failure to adhere to the therapy agreement, including attendance and payment policies.
Please note any threats or acts of violence will invalidate this agreement and therapy will cease. Sessions will not take place if you arrive under the influence of alcohol or non-prescribed medication.
Client Rights
You have the right to be treated with respect and dignity.
You have the right to ask questions and receive clear answers.
You have the right to confidentiality within the limits of the law.
You have the right to make decisions about your therapy.

By signing this agreement, you acknowledge that you have read, understood, and agree to the terms outlined above. If you have any questions or concerns, please discuss them with Dr Hannah Jenkins.
